Collect an application form from your nearest South Australian public library.

Once completed, the form must be signed in front of an independent adult (aged 18 or over) who is not related to you. This person will need to verify your identity by sighting documents such as your birth certificate, driver’s licence, passport, or similar proof of age. If you have all your documentation, a Justice of the Peace or an officer at your local police station should be able to help.
